Attach:
👉 Certification Letter issued by Chairman/Councilor/Mayor
👉 House Tax Receipt
(Alternatively, you can also write: Property Tax Receipt)A photocopy of your National ID (NID)
Proof of your new address (utility bill, rent receipt, etc.)
Submit the completed form to your local Election Commission Office
